The consent form
Until midnight your ID is enough. If you want to stay longer, a parent or guardian authorises an adult companion to look after you. That’s what this form is for.

- Everyone needs ID. ID card, passport or residence permit in the original. Student IDs or photos on your phone don’t count.
Three steps
- 1Get the templateOpen the template and print it. Write the event and the date on it.
- 2Get it signedYour parent or guardian and your companion sign it. Add a copy of your parent’s ID.
- 3Bring itThe form, the ID copy and your own ID. Your companion comes to the door with you and their ID.
For the companion
Who can come along?
- At least 18 years old and on site with their own ID.
- Stays with you the whole time and goes home with you.
- Accompanies at most two young people at once.
- Is sober enough to really look after you. We check that at the door.

Questions about the form
No. We need the signed paper and a copy of the signing person’s ID.
Filling in yes, but the signature has to be on it already. Without a signature you have to leave at midnight.
Then you leave too. If you stay on your own, we have to ask you to leave after midnight.
No, for one event on one date. For the next party you fill in a new one.
No. What counts is your age on the day of the party. Bring your ID.
